inclusions from crypto/ctr.h.

crypto_ctr_encrypt_walk() is no longer used so remove it.
Note that some existing drivers currently rely on the transitive
includes of some other crypto headers so retain those for the time
being.

Mark the src.virt.addr field in struct skcipher_walk as a pointer
to const data. This guarantees that the user won't modify the data
which should be done through dst.virt.addr to ensure that flushing
is done when necessary.

Add a static inline helper modeled after crypto_cbc_encrypt_walk()
that can be reused for SIMD algorithms that need to implement a
non-SIMD fallback for performing CTR encryption.

As discussed previously, this patch moves the basic CTR functionality
into a chainable algorithm called ctr. The IPsec-specific variant of
it is now placed on top with the name rfc3686.
So ctr(aes) gives a chainable cipher with IV size 16 while the IPsec
variant will be called rfc3686(ctr(aes)). This patch also adjusts
gcm accordingly.
