Upon receiving the Reset Request, pause the connection and clean up
queues, wait for the specified period, then resume the NIC.
In the cleanup phase, the HWC is no longer responding, so set hwc_timeout
to zero to skip waiting on the response.

MANA supports RDMA in PF mode. The driver should record the doorbell
physical address when in PF mode.
The doorbell physical address is used by the RDMA driver to map
doorbell pages of the device to user-mode applications through RDMA
verbs interface. In the past, they have been mapped to user-mode while
the device is in VF mode. With the support for PF mode implemented,
also expose those pages in PF mode.

In this patchset we want to enable the MANA driver to be able to
allocate MSI-X vectors in PCI dynamically.
The first patch exports pci_msix_prepare_desc() in PCI to be able to
correctly prepare descriptors for dynamically added MSI-X vectors.
The second patch adds the support of dynamic vector allocation in
pci-hyperv PCI controller by enabling the MSI_FLAG_PCI_MSIX_ALLOC_DYN
flag and using the pci_msix_prepare_desc() exported in first patch.
The third patch adds a detailed description of the irq_setup(), to
help understand the function design better.
The fourth patch is a preparation patch for mana changes to support
dynamic IRQ allocation. It contains changes in irq_setup() to allow
skipping first sibling CPU sets, in case certain IRQs are already
affinitized to them.
The fifth patch has the changes in MANA driver to be able to allocate
MSI-X vectors dynamically. If the support does not exist it defaults to
older behavior.

Currently, the MANA driver allocates MSI-X vectors statically based on
MANA_MAX_NUM_QUEUES and num_online_cpus() values and in some cases ends
up allocating more vectors than it needs. This is because, by this time
we do not have a HW channel and do not know how many IRQs should be
allocated.
To avoid this, we allocate 1 MSI-X vector during the creation of HWC and
after getting the value supported by hardware, dynamically add the
remaining MSI-X vectors.

In order to prepare the MANA driver to allocate the MSI-X IRQs
dynamically, we need to enhance irq_setup() to allow skipping
affinitizing IRQs to the first CPU sibling group.
This would be for cases when the number of IRQs is less than or equal
to the number of online CPUs. In such cases for dynamically added IRQs
the first CPU sibling group would already be affinitized with HWC IRQ.

Commit 91bfe210e196 ("net: mana: add a function to spread IRQs per CPUs")
added the irq_setup() function that distributes IRQs on CPUs according
to a tricky heuristic. The corresponding commit message explains the
heuristic.
Duplicate it in the source code to make available for readers without
digging git in history. Also, add more detailed explanation about how
the heuristics is implemented.

To collaborate with hardware servicing events, upon receiving the special
EQE notification from the HW channel, remove the devices on this bus.
Then, after a waiting period based on the device specs, rescan the parent
bus to recover the devices.

Check PF capability flag whether the 4M, 1G, and 2G pages are
supported. Add these pages sizes to mana_ib, if supported.
Define possible page sizes in enum gdma_page_type and
remove unused enum atb_page_size.

According to GDMA protocol, holes (zeros) are allowed at the beginning
or middle of the gdma_list_devices_resp message. The existing code
cannot properly handle this, and may miss some devices in the list.
To fix, scan the entire list until the num_of_devs are found, or until
the end of the list.

When on a MANA VM hibernation is triggered, as part of hibernate_snapshot(),
mana_gd_suspend() and mana_gd_resume() are called. If during this
mana_gd_resume(), a failure occurs with HWC creation, mana_port_debugfs
pointer does not get reinitialized and ends up pointing to older,
cleaned-up dentry.
Further in the hibernation path, as part of power_down(), mana_gd_shutdown()
is triggered. This call, unaware of the failures in resume, tries to cleanup
the already cleaned up mana_port_debugfs value and hits the following bug:

In mana_driver_exit(), mana_debugfs_root gets cleanup before any of it's
children (which happens later in the pci_unregister_driver()).
Due to this, when mana driver is configured as a module and rmmod is
invoked, following stack gets printed along with failure in rmmod command.

added memory allocation in mana_gd_setup_irqs of 'irqs' but the code
doesn't free this temporary array in the success path.

Implement debugfs in MANA driver to be able to view RX,TX,EQ queue
specific attributes and dump their gdma queues.
These dumps can be used by other userspace utilities to improve
debuggability and troubleshooting

A NULL dev->dma_parms indicates either a bus that is not DMA capable or
grave bug in the implementation of the bus code.
There isn't much the driver can do in terms of error handling for either
case, so just warn and continue as DMA operations will fail anyway.

As defined by the MANA Hardware spec, the queue size for DMA is 4KB
minimal, and power of 2. And, the HWC queue size has to be exactly
4KB.
To support page sizes other than 4KB on ARM64, define the minimal
queue size as a macro separately from the PAGE_SIZE, which we always
assumed it to be 4KB before supporting ARM64.
Also, add MANA specific macros and update code related to size
alignment, DMA region calculations, etc.

Process QP fatal events from the error event queue.
For that, find the QP, using QPN from the event, and then call its
event_handler. To find the QPs, store created RC QPs in an xarray.

Existing MANA design assigns IRQ to every CPU, including sibling
hyper-threads. This may cause multiple IRQs to be active simultaneously
in the same core and may reduce the network performance.
Improve the performance by assigning IRQ to non sibling CPUs in local
NUMA node. The performance improvement we are getting using ntttcp with
following patch is around 15 percent against existing design and
approximately 11 percent, when trying to assign one IRQ in each core
across NUMA nodes, if enough cores are present.
The change will improve the performance for the system
with high number of CPU, where number of CPUs in a node is more than
64 CPUs. Nodes with 64 CPUs or less than 64 CPUs will not be affected
by this change.
The performance study was done using ntttcp tool in Azure.
The node had 2 nodes with 32 cores each, total 128 vCPU and number of channels
were 32 for 32 RX rings.
The below table shows a comparison between existing design and new
design:

Souradeep investigated that the driver performs faster if IRQs are
spread on CPUs with the following heuristics:
1. No more than one IRQ per CPU, if possible;
2. NUMA locality is the second priority;
3. Sibling dislocality is the last priority.
Let's consider this topology:

for_each_numa_hop_mask() iterator and assigns IRQs to sibling groups

This patch allows to assign and poll more than one EQ on the same
msix index.
It is achieved by introducing a list of attached EQs in each IRQ context.
It also removes the existing msix_index map that tried to ensure that there
is only one EQ at each msix_index.
This patch exports symbols for creating EQs from other MANA kernel modules.

At present hwc timeout value is a fixed value. This patch sets the hwc
timeout from the hardware. It now uses a new hardware capability
GDMA_DRV_CAP_FLAG_1_HWC_TIMEOUT_RECONFIG to query and set the value
in hwc_timeout.

The hardware specification specifies that WQE_COUNT should set to 0 for
the Receive Queue. Although currently the hardware doesn't enforce the
check, in the future releases it may check on this value.

After calling irq_set_affinity_and_hint(), the cpumask pointer is
saved in desc->affinity_hint, and will be used later when reading
/proc/irq/<num>/affinity_hint. So the cpumask variable needs to be
persistent. Otherwise, we are accessing freed memory when reading
the affinity_hint file.
Also, need to clear affinity_hint before free_irq(), otherwise there
is a one-time warning and stack trace during module unloading:

To fix the bug, use the persistent mask, cpumask_of(cpu#), and set
affinity_hint to NULL before freeing the IRQ, as required by free_irq().

The PCI and queue number info is missing in IRQ names.
Add PCI and queue number to IRQ names, to allow CPU affinity
tuning scripts to work.

RDMA device may need to create Ethernet device queues for use by Queue
Pair type RAW. This allows a user-mode context accesses Ethernet hardware
queues. Export the supporting functions for use by the RDMA driver.
