Make the dp/hdmi_audio_* callback maintain the same parameter order as
get_modes and edid_read: first the bridge, then the connector.

Use new drm_hdmi_acr_get_n_cts() helper instead of hand-coding the
tables. Instead of storing the rate 'index', store the audio sample rate
in hdmi->audio.rate, removing the need for even more defines.

In order to simplify the driver even further and to remove the
boilerplate code, rewrite the audio interface to use the DRM HDMI Audio
framework.
Audio InfoFames are controlled centrally via the DRM HDMI framework.
Correct InfoFrame data is programmed at the atomic_pre_enable() time (if
it was set before, drm_atomic_helper_connector_hdmi_update_infoframes()
takes care of writing all InfoFrames, including the Audio one.) or
during msm_hdmi_bridge_audio_prepare() when the new stream is started.
All audio data frame management is deferred to
msm_hdmi_bridge_audio_prepare() and msm_hdmi_bridge_audio_shutdown().

Commit c0c0d9eeeb8d ("drm/msm: hdmi audio support") uses logical
OR operators to build up a value to be written in the
REG_HDMI_AUDIO_INFO0 and REG_HDMI_AUDIO_INFO1 registers when it
should have used bitwise operators.

Global symbols in the kernel should be prefixed by the name
of the subsystem and/or driver to avoid conflicts when all
code is built-in.
In this case, function names like 'hdmi_register' or 'hdmi_set_mode'
are way too generic for an MSM specific DRM driver, so I'm renaming
them all to msm_hdmi_* here.
I also rename a lot of the 'static' symbols along with the global
names for consistency, even though those are relatively harmless;
they might only be slightly confusing when they show up in
backtraces.
