ARM already has an arm/ subdirectory, but s390 and x86 do not even though
they have a relatively large number of files specific to them. Create
new directories in Documentation/virt/kvm for these two architectures
as well.
While at it, group the API documentation and the developer documentation
in the table of contents.

Nested VMX was enabled by default in commit 1e58e5e59148 ("KVM:
VMX: enable nested virtualization by default"), which was merged
in Linux 4.20. This patch is to fix the documentation accordingly.

This is a rewrite of this[1] Wiki page with further enhancements. The
doc also includes a section on debugging problems in nested
environments, among other improvements.
