2023-05-31scsi: lpfc: Change firmware upgrade logging to KERN_NOTICE instead of ↵Justin Tee
TRACE_EVENT A firmware upgrade does not necessitate dumping of phba->dbg_log[] to kmsg via LOG_TRACE_EVENT. A simple KERN_NOTICE log message should suffice to notify the user of successful or unsuccessful firmware upgrade. As such, firmware upgrade log messages are updated to use KERN_NOTICE instead of LOG_TRACE_EVENT. Additionally, in order to notify the user of reset type for instantiating newly downloaded firmware, lpfc_log_msg's default KERN_LEVEL is updated to 5 or KERN_NOTICE. 2023-05-08scsi: lpfc: Replace blk_irq_poll intr handler with threaded IRQJustin Tee
It has been determined that the threaded IRQ API accomplishes effectively the same performance metrics as blk_irq_poll. As blk_irq_poll is mostly scheduled by the softirqd and handled in softirq context, this is not entirely desired from a Fibre Channel driver context. A threaded IRQ model fits cleaner. This patch replaces the blk_irq_poll logic with threaded IRQ. 2023-04-11scsi: lpfc: Fix ioremap issues in lpfc_sli4_pci_mem_setup()Shuchang Li
When if_type equals zero and pci_resource_start(pdev, PCI_64BIT_BAR4) returns false, drbl_regs_memmap_p is not remapped. This passes a NULL pointer to iounmap(), which can trigger a WARN() on certain arches. When if_type equals six and pci_resource_start(pdev, PCI_64BIT_BAR4) returns true, drbl_regs_memmap_p may has been remapped and ctrl_regs_memmap_p is not remapped. This is a resource leak and passes a NULL pointer to iounmap(). To fix these issues, we need to add null checks before iounmap(), and change some goto labels. 2023-03-09scsi: lpfc: Drop redundant pci_enable_pcie_error_reporting()Bjorn Helgaas
pci_enable_pcie_error_reporting() enables the device to send ERR_* Messages. Since commit f26e58bf6f54 ("PCI/AER: Enable error reporting when AER is native"), the PCI core does this for all devices during enumeration, so the driver doesn't need to do it itself. Remove the redundant pci_enable_pcie_error_reporting() call from the driver. Also remove the corresponding pci_disable_pcie_error_reporting() from the driver .remove() path. Note that this only controls ERR_* Messages from the device. An ERR_* Message may cause the Root Port to generate an interrupt, depending on the AER Root Error Command register managed by the AER service driver. 2023-03-09scsi: lpfc: Skip waiting for register ready bits when in unrecoverable stateJustin Tee
During tolerance tests that force an HBA to become unresponsive, rmmod hangs resulting in the inability to remove the driver. The lpfc_pci_remove_one_s4() routine attempts to submit a clean up mailbox command via the lpfc_sli4_post_sync_mbox() routine, but ends up waiting forever for a mailbox register to set its ready bit. Because the HBA is in an unrecoverable and unresponsive state, the ready bit will never be set. Create a new routine called lpfc_sli4_unrecoverable_port(), which checks a port status register's error notification bits. Use the lpfc_sli4_unrecoverable_port() routine in ready bit check routines to early return error if port is deemed unrecoverable. Also, when the lpfc_handle_eratt_s4() handler detects an unrecoverable state, call the lpfc_sli4_offline_eratt() routine to kick off flushing outstanding I/O. 2023-03-06cpumask: fix incorrect cpumask scanning result checksLinus Torvalds
It turns out that commit 596ff4a09b89 ("cpumask: re-introduce constant-sized cpumask optimizations") exposed a number of cases of drivers not checking the result of "cpumask_next()" and friends correctly. The documented correct check for "no more cpus in the cpumask" is to check for the result being equal or larger than the number of possible CPU ids, exactly _because_ we've always done those constant-sized cpumask scans using a widened type before. So the return value of a cpumask scan should be checked with if (cpu >= nr_cpu_ids) ... because the cpumask scan did not necessarily stop exactly *at* that maximum CPU id. But a few cases ended up instead using checks like if (cpu == nr_cpumask_bits) ... which used that internal "widened" number of bits. And that used to work pretty much by accident (ok, in this case "by accident" is simply because it matched the historical internal implementation of the cpumask scanning, so it was more of a "intentionally using implementation details rather than an accident"). But the extended constant-sized optimizations then did that internal implementation differently, and now that code that did things wrong but matched the old implementation no longer worked at all. Which then causes subsequent odd problems due to using what ends up being an invalid CPU ID. Most of these cases require either unusual hardware or special uses to hit, but the random.c one triggers quite easily. All you really need is to have a sufficiently small CONFIG_NR_CPUS value for the bit scanning optimization to be triggered, but not enough CPUs to then actually fill that widened cpumask. At that point, the cpumask scanning will return the NR_CPUS constant, which is _not_ the same as nr_cpumask_bits. This just does the mindless fix with sed -i 's/== nr_cpumask_bits/>= nr_cpu_ids/' to fix the incorrect uses. The ones in the SCSI lpfc driver in particular could probably be fixed more cleanly by just removing that repeated pattern entirely, but I am not emptionally invested enough in that driver to care. Define new FC Link ACQE with new attention types 0x8 (Link Activation Failure) and 0x9 (Link Reset Protocol Event). Both attention types are meant to be informational-only type ACQEs with no action required. 0x8 is reported for diagnostic purposes, while 0x9 is posted during a normal link up transition when activating BB Credit Recovery feature. As such, modify lpfc_sli4_async_fc_evt() logic to log the attention types according to its severity and early return when informational-only attention types are encountered. 2022-11-17scsi: lpfc: Fix MI capability display in cmf_info sysfs attributeJustin Tee
The dynamic mi_ver value holds the currently configured MI setting. mi_ver was being displayed as part of the cmf_info sysfs attribute, when the output string meant to display MI capabilities instead. Add a mi_cap member in the lpfc_pc_sli4_params structure that will store MI capabilities during initialization so that cmf_info prints out capabilities instead of current configuration. 2022-11-17scsi: lpfc: Fix WQ|CQ|EQ resource checkJustin Tee
Adapter configurations with limited EQ resources may fail to initialize. Firmware resources are queried in lpfc_sli4_read_config(). The driver parameters cfg_irq_chann and cfg_hdw_queue are adjusted from defaults if constrained by firmware resources. The minimum resource check includes a special allocation for queues such as ELS, MBOX, NVME LS. However the additional reservation was also incorrectly applied to EQ resources. Reordered WQ|CQ|EQ resource checks to apply the special allocation adjustment to WQ and CQ resources only. 2022-10-18scsi: lpfc: Fix memory leak in lpfc_create_port()Rafael Mendonca
Commit 5e633302ace1 ("scsi: lpfc: vmid: Add support for VMID in mailbox command") introduced allocations for the VMID resources in lpfc_create_port() after the call to scsi_host_alloc(). Upon failure on the VMID allocations, the new code would branch to the 'out' label, which returns NULL without unwinding anything, thus skipping the call to scsi_host_put(). Fix the problem by creating a separate label 'out_free_vmid' to unwind the VMID resources and make the 'out_put_shost' label call only scsi_host_put(), as was done before the introduction of allocations for VMID. 2022-09-15scsi: lpfc: Move scsi_host_template outside dynamically allocated/freed phbaJames Smart
On a PCI hotplug capable system, it is possible for scsi_device_put() to happen after lpfc_pci_remove_one() is called. As a result, the sdev->host->hostt->module dereference is for a previously freed memory location because the phba structure containing the hostt template was already freed when lpfc_pci_remove_one() returned. Since the lpfc module is still loaded during power slot disable, all scsi_host_templates should be declared as part of the global data segment instead of inside the heap allocated phba structure. This way the sdev->host->hostt memory area is always valid as long as the module is loaded regardless if PCI hotplug dynamically allocates or frees phba structures. Move all scsi_host_templates in the phba structure to global variables. Create a small helper routine to determine appropriate sg_tablesize during shost allocation. Thus, the macro usage of LPFC_RXMONITOR_TABLE_IN_USE can be unintentionally truncated leading to all logic involving the LPFC_RXMONITOR_TABLE_IN_USE macro to not work properly. Replace the Rx Table atomic_t indexing logic with a new lpfc_rx_info_monitor structure that holds a circular ring buffer. For locking semantics, a spinlock_t is used. After a link up, it's possible for the switch to change FDMI support (e.g. FDMI1 vs FDMI2 vs SmartSAN). If the switch reverts to FDMI1, then the revert is currently not detected. Additionally, when NPIV is configured, it's possible the physical port's RHBA is unprocessed by the switch before reciept of an NPIV port issued RPRT. This causes some switches vendors to reject the NPIV's RPRT. Fix by reinitializing base FDMI mode on link up, and defer FDMI vport RPRT submission until after confirming physical port's RHBA is completed. Do not rely on vendor version field of the CSPs to determine if we are in a FA-PWWN environment. Instead, use the following procedure: First, during HBA initialization, driver does a READ_CONFIG to determine if FA-PWWN is configured on the HBA. A LPFC_FAWWPN_CONFIG hba_flag is set accordingly. Next, when the link comes up before the driver gets a link up event, the firmware logs into the fabric with FA-PWWN. If the fabric port does not support FA-PWWN, the driver will get a Misconfigured FA-WWN async event before the link up. A LPFC_FAWWPN_FABRIC hba_flag will be set accordingly. Finally, if the fabric supports FA-PWWN, the firmware will replace its CSPs WWN with the Fabric Assigned ones. Then after link up, the driver will retrieve the Fabric Assigned WWN when it does a READ_SPARAM mbox command. The intention of this patch is to refactor mailbox memory allocation and cleanup steps in one routine respectively to prevent memory leaks or memory errors related to mailbox commands. There are trivial localized fixes as well. Provide lpfc_mbox_rsrc_prep() - this routine allocates the dmabuf and the mbuf associated with it. It also catches allocation errors and returns status. Provide lpfc_mbox_rsrc_cleanup() - this routine verifies a dmabuf exists and if so releases the associated mbuf and the dmabuf memory. It then sets the ctx_buf to NULL and releases the mailbox memory to the mailbox pool. The lpfc_iocbq data structure has void * pointers that are overloaded to be as many as 8 different data types and the driver translates the void * by casting. This patch removes the void * pointers by declaring the specific types needed by the driver. It also expands the context_un to include more seldom used pointer types to save structure bytes. It also groups the u8 types together to pack the 8 bytes needed. This work allows the lpfc_iocbq data structure to be more strongly typed and keeps it from being allocated from the 512 byte slab. 2022-03-29scsi: lpfc: Fix queue failures when recovering from PCI parity errorJames Smart
When recovering from a pci-parity error the driver is failing to re-create queues, causing recovery to fail. Looking deeper, it was found that the interrupt vector count allocated on the recovery was fewer than the vectors originally allocated. This disparity resulted in CPU map entries with stale information. When the driver tries to re-create the queues, it attempts to use the stale information which indicates an eq/interrupt vector that was no longer created. Fix by clearng the cpup map array before enabling and requesting the IRQs in the lpfc_sli_reset_slot_s4 routine(). 2022-03-29scsi: lpfc: Fix unload hang after back to back PCI EEH faultsJames Smart
When injecting EEH errors the port is getting hung up waiting on the node list to empty, message number 0233. The driver is stuck at this point and also can't unload. The driver makes transport remoteport delete calls which try to abort I/O's, but the EEH daemon has already called the driver to detach and the detachment has set the global FC_UNLOADING flag. There are several code paths that will avoid I/O cleanup if the FC_UNLOADING flag is set, resulting in transports waiting for I/O while the driver is waiting on transports to clean up. Additionally, during study of the list, a locking issue was found in lpfc_sli_abort_iocb_ring that could corrupt the list. A special case was added to the lpfc_cleanup() routine to call lpfc_sli_flush_rings() if the driver is FC_UNLOADING and if the pci-slot is offline (e.g. EEH). The SLI4 part of lpfc_sli_abort_iocb_ring() is changed to use the ring_lock. Also added code to cancel the I/Os if the pci-slot is offline and added checks and returns for the FC_UNLOADING and HBA_IOQ_FLUSH flags to prevent trying to send an I/O that we cannot handle. 2022-03-29scsi: lpfc: Improve PCI EEH Error and Recovery HandlingJames Smart
Following EEH errors, the driver can crash or hang when deleting the localport or when attempting to unload. The EEH handlers in the driver did not notify the NVMe-FC transport before tearing the driver down. This was delayed until the resume steps. This worked for SCSI because lpfc_block_scsi() would notify the scsi_fc_transport that the target was not available but it would not clean up all the references to the ndlp. The SLI3 prep for dev reset handler did the lpfc_offline_prep() and lpfc_offline() calls to get the port stopped before restarting. The SLI4 version of the prep for dev reset just destroyed the queues and did not stop NVMe from continuing. Also because the port was not really stopped the localport destroy would hang because the transport was still waiting for I/O. Additionally, a devloss tmo can fire and post events to a stopped worker thread creating another hang condition. lpfc_sli4_prep_dev_for_reset() is modified to call lpfc_offline_prep() and lpfc_offline() rather than just lpfc_scsi_dev_block() to ensure both SCSI and NVMe transports are notified to block I/O to the driver. Logic is added to devloss handler and worker thread to clean up ndlp references and quiesce appropriately.
