We need to take the manageability semaphore when issuing firmware
commands to avoid problems. With this in place, the semaphore is
no longer taken in the ixgbe_set_fw_drv_ver_generic function, since
it will now always be taken by the ixgbe_host_interface_command
function.

Clean up the interface for issuing firmware commands to use a
void * instead of a u32 *. This eliminates a number of casts.
Also clean up ixgbe_host_interface_command in a few other ways,
eliminating comparisons with 0, redundant parens and minor
formatting issues.

The function ixgbe_host_interface_command actually uses a multiple
of word sized buffer to do its business, but only checks against
the actual length passed in. This means that on read operations it
could be possible to modify locations beyond the length passed in.
Change the check to round up in the same way, just to avoid any
possible hazard.

This patch makes certain that we clear the pool mappings added when we
configure default MAC addresses for the interface. Without this we run the
risk of leaking an address into pool 0 which really belongs to VF 0 when
SR-IOV is enabled.

This patch is meant to reduce the complexity of the search function used
for finding a VLVF entry associated with a given VLAN ID. The previous
code was searching from bottom to top. I reordered it to search from top
to bottom. In addition I pulled an AND statement out of the loop and
instead replaced it with an OR statement outside the loop. This should
help to reduce the overall size and complexity of the function.
There was also some formatting I cleaned up in regards to whitespace and
such.

This patch adds support for bypassing the VLVF entry creation when the PF
is adding a new VLAN. The advantage to doing this is that we can then save
the VLVF entries for the VFs which must have them in order to function,
versus the PF which can fall back on the default pool entry.

This patch addresses several issues within the VLVF and VLVFB
configuration
First was the fact that code was overly complicated with multiple
conditional paths depending on if we adding or removing and which bit we
were going to add or remove. Instead of messing with all that I have
simplified it by using (vid / 32) and (1 - vid / 32) to identify our
register and the other vlvfb register.
Second was the fact that we were likely leaking a few packets into the PF
in cases where we were deleting an entry and the VFTA filter for that entry
as the ordering was such that we deleted the pool and then the VLAN filter
instead of the other way around. I have updated that by adding a check for
no bits being set and if that occurs we clear things up in the proper
order.

In order to clear the way for upcoming work I thought it best to drop the
level of indent in the ixgbe_set_vfta_generic function. Most of the code
is held in the virtualization specific section. So the easiest approach is
to just add a jump label and jump past the bulk of the code if it is not
enabled.

This patch simplifies the logic for setting the VFTA register by removing
the number of conditional checks needed. Instead we just use some boolean
logic to generate vfta_delta, and if that is set then we xor the vfta by
that value and write it back.

The 82599 and X540 datasheets require that FCRTH be "set" for Tx
switching (VM-to-VM loopback) but it did not previously specify what
the value should be set to. It has now been determined that
the correct value is RXPBSIZE - (24*1024).
This setting is also required for later devices.

This patch skips the PCI transactions pending check in
ixgbe_disable_pcie_master. This is done to addresses a known HW
issue where the PCI transactions pending bit sticks high when there
are pending transactions. HW engineering instructed to workaround
this issue by wait and then continue with our reset flow.

These Device ID could support both WoL and autoneg flow control. In
the case of WoL this is indicated by the eeprom. This patch enables
these devices this support.

Some of the register addresses and format where unfortunately changed
between MAC types. To get around this we add a const u32 *mvals pointer
to the ixgbe_hw struct to point to an array of mac-type-dependent
values. These can include register offsets, masks, whatever can be in
a u32. When the ixgbe_hw struct is initialized, a pointer to the
appropriate array must be set.

For the X550 mac type we have to do additional steps around
enabling/disabling Rx. This patch will add a layer of indirection

This patch extends the function pointer structure to include the new
X550 class MAC types. This creates a new file ixgbe_x550.c that contains
all of the new methods. Because of similarities to the X540 part in
some cases we just use it's methods where they can be used without any
modification. These exported functions are now defined in the new
ixgbe_x540.h file.

Currently the shared code checksum calculation function only
returns a u16 and cannot return an error code. Unfortunately
a variety of errors can happen that completely prevent the
calculation of a checksum. So, change the function return value
from a u16 to an s32 and return a negative value on error, or the
positive checksum value when there is no error.

The X550 hardware will use more bits in the mask, so change
the prototypes to match. This larger mask will require changes
in callers which use the higher bits. Likewise since X550 will
use different semaphore mask values and will use the lan_id
value. So save these values in the ixgbe_phy_info struct.

Since on X550 we use host interface commands to read,write and erase
some commands require more time to complete. So this adds a timeout
parameter to ixgbe_host_interface_command as wells as a return_data
parameter allowing us to return with any data.

On topologies including few levels of PCIe switching X540 can run into an
unexpected completion error. We get around this by waiting after enabling
loopback a sufficient amount of time until Tx Data Fetch is sent. We then
poll the pending transaction bit to ensure we received the completion. Only
then do we go on to clear the buffers.

Since fc.high_water is an array, we should treat low_water as an array
also. This allows the algorithm to output different values for different
TCs, and then we can distinguish between them. In addition, this patch
changes one path that didn't honor the return value from ixgbe_setup_fc.

When an adapter is removed and registers all read as all one's,
an infinite recursion can happen between ixgbe_clear_vmdq_generic
and ixgbe_clear_rar_generic. Adding a check for removal breaks
this recursion.

This patch replaces some direct uses of pci_read_config_word with the
protected ixgbe_read_pci_cfg_word function, which checks for whether the
adapter is removed when LER is enabled. We shouldn't use the
pci_read_config_word calls directly because of these checks.
This patch also cleans up an unnecessary save of a pointer to the mac
object, as our standard style is to just use the hw pointer.

This patch reverts the addition of the fiber_fixed type, which ended up
never being used. We don't have plans to support this type going
forward, and there is no reason to keep an unused type around polluting
the code.

Found several incorrect conditionals after calling the prot_autoc_*
functions. Likewise we weren't always freeing the FWSW semaphore after
grabbing it. This would lead to DA cables being unable to link along with
possible other errors.

When reading or writing to the AUTOC register on 82599 devices we need to
preform various operations that aren't needed for other MAC types. This
patch will collect all of that code into one place to minimize MAC checks
in common code paths.
While doing this I also clean up some cases where we weren't holding the
SW/FW semaphore during a read/modify/write of AUTOC.

Currently we were just always polling for a hard coded 80 ms and not
respecting the system-wide timeout interval. Since up until now all
devices have been tested with this 80ms value we continue to use this
value as a hard minimum.

This patch fixes the possible use of uninitialized memory by checking the
return value on eeprom reads. These issues were identified by static
analysis. In many cases error messages will be produced so that corrupted
eeprom issues will be more visible.

This patch changes the error code path in ixgbe_acquire_swfw_sync() to deal
with cases where acquiring SW semaphore times out.
In cases where the SW/FW semaphore bits were set (i.e. due to a crash) the
driver will hang on load. With this patch the driver will clear
the stuck bits if the semaphore was not acquired in the allotted time.

This patch adds support for a new media type fiber_fixed. This is useful
to avoid all the SFP+ hot plug support path on devices who's fix fiber need
not worry about such things. This patch is needed for a following patch
that adds support for "fiber_fixed" devices.

Originally ixgbe_device_supports_autoneg_fc() was only expected to
be called by copper devices. This would lead to false information
to be displayed via ethtool.
