This patch applies customized PCI_DEVICE_ macros to specify the pci_ids
instead of open-coding them within the sdhci-pci driver.
By introducing device specific macros the pci_ids table becomes much
shorter and easier to comprehend than it would be possible using the
generic version of the PCI_DEVICE_ macros.

The suspend / resume callbacks lack the flexibility to allow a device to
specify a different function entirely. Change them around so that device
functions are called directly and they in turn can call the default
implementations if needed.

SDHCI provides more flexibility than simply calling sdhci_add_host(). Make
that available by allowing devices to specify their own ->add_host()
function.

Let devices define their own private data to facilitate device-specific
operations. The size of the private structure is specified in the
sdhci_pci_fixes structure, then sdhci_pci_probe_slot() will allocate extra
space for it, and sdhci_pci_priv() can be used to get a reference to it.

Devices might save and restore tuning values so that re-tuning might not be
needed after a pm transition. Let drivers decide by pushing the
mmc_retune_needed() logic down to them.

The commit 52ac7acf412b ("mmc: sdhci-pci: Convert to use managed functions
pcim_* and devm_*") converted ->probe() / ->remove() functions to use device
managed resource API. Here is a follow up to cover sdhci_pci_probe_slot() and

Add support and PCI IDs for more Broxton host controllers
Other BXT IDs were added in v4.4 so cc'ing stable. This patch
is dependent on commit 163cbe31e516 ("mmc: sdhci-pci: Fix card
detect race for Intel BXT/APL") but that is already in stable
since v4.4.4.

Add support for card detect for Bay Trail
and Braswell SD Card host controllers in PCI
mode.
This uses the gpio descriptor API which can find
gpio descriptors, for example, on an ACPI comapnion
device.

A host controller for a SD card may need a GPIO for card detect in order
to wake up from runtime suspend when a card is inserted. If that GPIO is
not configured, then the host controller will not wake up. Fix that for
the affected devices by not enabling runtime PM unless the GPIO is
successfully set up.
This affects BYT sd card host controller which had runtime PM enabled from
v3.11. For completeness, the MFD sd card host controller is flagged also.
The original patch before rebasing (see link below) was tested on v3.11.10
and v3.12.4 although the patch applied with some offsets and fuzz. The
original patch is here:
